If you have Adobe Acrobat save the file on your computer open it with Acrobat, Click on Tools > Typewriter, then fill the form in, save it and then email it to us.
If you have Microsoft Word save the file on your computer open it with Word, fill in the form, save it and then email it to us.
You will be able to sign the form when you first attend the club.
If you have neither of the above Programs open the Adobe file with Adobe
Reader (Free
Download), print the form, fill it in, sign it and bring it with you
to the club.
